Patton ko achhi tarah dho lein. Ek litre paani me daal kar 10 minute ubaalein, phir chhaan kar gungune ya thanda hone tak rakhein - garam paani ko bachche ke paas na rakhein, gir kar jal jaane ka khatra hai. Wash the leaves well. Boil them in a litre of water for about 10 minutes, strain, and let the water cool to lukewarm or cold - keep the hot pot away from children, a spill scalds. Before the first use, dab some on your wrist and wait 24 hours. Wash the itchy area gently with the water, or tip it into a bucket of bath water; rinse afterwards with plain water and moisturise. Throw the leftover water away rather than keeping it for the next day. This water is not for drinking, and neem oil or leaf juice must never be swallowed - in small children it has been fatal.

Neem is trusted enough in Indian homes that people call it 'the household physician' - a figure of speech, not a stand-in for a doctor. In chickenpox, boils, pimples and itching the leaves are laid on the bed or dropped into bath water. The claim is that neem cleans the blood and settles heat rashes. Ayurvedic texts have mentioned it for centuries.

We found no decent human trial of neem for pimples or ordinary itching. The study usually quoted is a 1992 pilot in Tropical and Geographical Medicine on scabies - a neem and turmeric paste was applied to 814 people and the authors claimed the rash cleared in 97% of them within 3 to 15 days, but the abstract reports no control or comparison group at all, so there is no way to tell whether the paste or time did the work. It is also a study of scabies, a mite infestation, so nothing about acne or everyday itching follows from it. Wikipedia's Azadirachta indica page states there is insufficient clinical evidence for medicinal benefit. So: an old habit research has not got round to testing properly. The risk, though, is real - NIH's LiverTox records severe metabolic acidosis, coma and deaths in small children who swallowed neem oil, including a 4-month-old who died of cerebral edema.
